Patients who are of Aboriginal and/or Torres Strait Islander descent are at a greater risk for developing preventable chronic conditions or illness.
At Belgrave Medical Clinic we offer a free Health Assessment service which can help identify your risks of developing chronic conditions or illness and provide recommendations to help reduce your risk.
The Assessment includes:
- A one-hour appointment with the practice nurse and doctor at no cost to you.
- Undertaking examinations including blood pressure, height and weight.
- Assessing disease risk factors such as diabetes, cardiovascular disease, cancer and more.
- Arranging appropriate tests to assess, monitor and manage your health.
- Allied Health referral eligibility. Medicare will help to fund up to 10 visits with Allied Health professionals per calendar year.
- Review of immunisation status and recommendations.
- Review of eligible cancer screening and recommendations.
- Setting personalised health goals.
- Education and prevention advice.
After the assessment:
- A plan will be made about your ongoing care, follow up appointments with the GP may be required to review and discuss results.
- Patients are entitled to free nurse follow up to help you manage your ongoing health, these can include: immunisations, following up on recommendations made by the doctor, monitoring health conditions, weight/blood pressure checks and supporting you with the goals you’ve set.
Eligibility:
- All people of Aboriginal and/or Torres Strait Islander descent are eligible for a Health Assessment every 9 months.
